Yes it is rude. You can provide your transcript and the prof can decide what that means if anything

You cannot ask a letter writer to write anything. What you can do is just provide the information yourself, and if he believes it is worth mentioning, he will. Never imply anything about what should go into the letter. You are not a judge of that, and neither are you impartial.

You dosing get to dictate what goes in the letter, but you should try to get a sense of whether it will be a strong letter or not. Usually if it won't be, the professor will express some hesitation and claim that s/he "doesn't know you well enough" or something. If this happens you need to find a new letter writer immediately.
I think it’s totally fine to give a short list of things that you’re concerned about. So you want to frame it as “it’s your letter, of course write what you want, but I’m worried about X and Y so If you’re willing to address them that might help me.” No one will get mad about that, and some people appreciate the guidance on important things to address.
