I had an interview a month ago, so at this point I'm sure I'm not the top candidate. I'm second-guessing some of my responses. Maybe I was too casual. Y'all think that stuff makes a difference, or is it usually substance?

Some places fly out much more than 5-7. Yale typically does 15+. Last year Penn flew out like 20.
That said, for the average program in the 20-50 range, you’re probably right. Often there is a relative consensus about who those top 5-7 candidates are. If their interviews go without incident, they are flown out. If their interviews go sideways, then there’s a discussion of who replaces them. That happens in a shorter amount of time and there may not have been a consensus beforehand on who the next best candidate is. Also, flyouts are expensive (in terms of faculty time). Therefore it is useful to gather a lot of data on the possible replacements during the interview stage.
I don’t understand this. Schools interview 30 people but fly out only 5-7 for one position.
Interviews can rarely help you. They can easily hurt you though. Just don’t f it up majorly and you’ll be fine.
