a > B. Having no flyouts suggests that you did not even meet the initial cutoff (which is typically lower than one for receiving an offer). A lot of things play into receiving an offer after flyouts and many of those may be beyond your control.

I know it’s normal meet the Dean and Associate Dean during the campus visit. Is it also normal for a rookie to have meals with Dean’s office admins at research universities? Should I read anything into that?
You should read into it that they couldn’t find anyone else to have lunch with you.

It's not common, but I wouldn't read too much into it. May have been a scheduling issue where no faculty were available, maybe they really value their admins, maybe they thought you'd get along well with a particular admin. You can drive yourself crazy over-analyzing every little thing while waiting for news. Everyone does this a bit, but it's not productive.

If you have more than one offers and it is very very difficult to choose between the two, can you ask for a second visit to each location with your spouse? Should you let them know you're coming? We're willing to pay the travel costs.
There are no rules. But I’ve seen this happen. Some schools understand that family matters.

That should be a CB offer. Their quant flyouts are still ongoing.
Who got the offer from Toronto? Is that TL from Chicago?

Heard it is a quant offer.
That doesn't make much sense -- so far they've only seen UN (accepted another offer) and OR from UWash. DA and TL haven't done their flyouts there yet.
