I used to think kids like these were mentally ill. Reading more of these stories have changed my perspective. The kids are perfectly normal kids. It's the parents that are mentally ill, and the kids have simply found the most efficient way to garner the attention they crave from their mentally-ill parents.

there are two genders, as determined by the genitalia at birth (except in a few cases where individuals are biologically intersex). A further small group of people have gender dysmorphia, which is a psychological condition.
btw it is mentioned that the child in the story has an autistic spectrum disorder - this has been shown to be correlated with gender confusion in previous research. so we are perhaps "treating" a symptom of autism by pumping people full of sex hormones and maiming them for life.
